You must submit the CCIDA Application form as well as the New York State’s short Environmental Assessment form (SEQR), as required by the State’s Environmental Quality Review Act (SEQR).
In New York State, most projects or activities proposed by a New York State agency or by a unit of local government, or which require discretionary approval from a N.Y.S agency or a unit of local government, are subject to the N.Y.S Environmental Quality Review Act (SEQR). These include projects sponsored by CCIDA.
Specifically, you must complete the Full Environmental Assessment Form (EAF).
These Applications are needed for the CCIDA to consider a project for inducement. When completed, please mail the Sale Leaseback Application and the Full EAF Form, along with a check for $1,500.00 made payable to CCIDA to the following address:

The Applications listed above along with the N.Y. State Environmental Assessment Forms (SEQR) forms are designed to be filled in on your computer, printed on your printer, signed and mailed/e-mailed to our offices.
